Would you like to expedite the registration process by printing your own forms? Simply print and complete the forms listed below. Once completed, you can do one of the following:

-Drop off forms at any Smart Steps location
Once your information is received,
you will be contacted to schedule a tour.
Note: Registration is just $20 per child. Checks and money orders are accepted.
HEALTH INVENTORY- Must be completed by a doctor showing a physical has been completed within the last 12 months. (Includes immunization information.)
- Must be completed by parent listing current contact information.
MEDICATION ADMINISTRATION FORM
- This form is required if parents want sites to administer medication to students while in care. Please read carefully. Each site has at least one person on duty at all times with a Medication Administration Certification.
- This form outlines center regulations, pricing, and specific parent needs. Note that prices are subject to change.
To secure your child's space, the above forms must be submitted along with a $20 registration fee and full payment for the first week of care (once a space has been assigned). Additionally, a parent handbook receipt and Meal Benefit Application will be issued during your tour. Space is limited to site capacity and ratio requirements. Secure your space now!
